      Prin acest document se confirma ca sunt indeplinite cerintele de calificare astfel cum au fost solicitate in documentatia de atribuire.Se vor completa ulterior documentele dupa cum urmeaza:

      — ofertantul are obligatia de a prezenta/completa certificatele/documentele edificatoare care probeaza/confirma indeplinirea cerintelor de calificare, atunci cand primeste din parte autoritatii contractante o solicitare in acest sens, in termenul prevazut in respectiva solicitare. Acest termen nu va fi mai mic de 3 zile lucratoare. Autoritatea contractanta se va asigura ca ofertantul a carei oferta este declarata castigatoare, a prezentat, nu mai tarziu de incheierea raportului procedurii de atribuire, certificatele/documentele solicitate.

      2. Modalitatea de departajare a ofertelor cu acelasi pret clasate pe primul loc: in cazul in care se constata ca ofertele clasate pe primul loc au preturi egale, autoritatea contractanta va solicita clarificari prin intermediul SEAP, in vederea incarcarii electronice de catre operatorii economici de documente care contin noi propuneri financiare imbunatatite.

      3. Raspunsurile la clarificarile solicitate de operatorii economici, notificari/instiintari referitoare la procedura de achizitie vor fi publicate in SEAP, atasate invitatiei de participare. Operatorii economici care intentioneaza sa depuna oferta, au obligatis de a urmari/accesa site-ul www.e-licitatie.ro pe toata durata de desfasurare a prezentei proceduri.

      4. Modalitatea de comunicare: fiind o procedura de achizitie desfasurata integral prin mijloace electronice prin intermediul platformei electronice administrata de SEAP, operatorii economici pot solicita clarificari privind doc de atribuire si pot transmite raspunsuri la solicitarile de clarificari ale autoritatii contractante, doar prin intermediul SEAP. Garantiile de participare se gasesc in sectiunea Informatii privind loturile pe fiecare lot in parte. In cazul in care se constata ca ofertele clasate pe primul loc au preturi egale, autoritatea contractanta va solicita clarificari prin intermediul SEAP, catre operatorii economici pentru departajare. In cazul demararii procedurii de licitatie centralizata de catre Ministerul Sanatatii, autoritatea contractanta isi rezerva dreptul de a anula aceasta procedura sau de a rezilia contractele pentru loturile licitate de ministerul sanatatii, atat in SEAP, cat si prin comunicare scrisa catre opertatorii economici.
